Gallbladder Torsion - A case report -

Abstract

|
Gallbladder torsion is a rare surgical emergency primarily occurring in elderly women. The cause of this condition is related to anatomical variation in the attachment of the gallbladder to the inferior margin of the liver. Because of its rarity,
the
correct preoperative diagnosis is seldom made and the patients usually come to surgery on suspicion of acute cholecystitis or appendicitis. A 67-year-old male patient was admitted at Holy Family Hospital, Catholic University Medical College, due
to
sudden right upper quadrant abdominal pain and vomiting. Abdominal ultrasound revealed a markedly distended gallbladder with a thickened wall which may be appeared to be corresponded with acalculous cholecystitis. Emergency explolaparotomy showed
necrotizing cholecystitis due to acute gallbladder torsion a 270 degree counterclockwise rotation around the cystic duct and artery. After cholecystectomy, the patient was discharged in good condition without any complications. In order to avoid
unnecessary complictions, this should be suspected in elderly patients with an acute onset of right upper quadrant abdominal pin or with nausea and vomiting.
